Background and Objectives: Breast cancer is the leading cause of death from cancer among women. In developing countries patients 'survival time is less than developed countries. Metastasis, as an intermediate event affected the survival time of breast cancer patients. This study aimed to evaluate the survival of patients after surgery by taking metastasis, as the intermediate event by using Illness-death model.
Methods: In this study 165 cases of Iranian breast cancer patients which had surgery over the 2006-2014 years were investigated. Patients’ characteristics, intermediate and final status were collected from medical records of the patients. Illness-death model was used to assessing prognostic factors on all stages of disease.
Results: The risk of metastasis in patients who have tumor size between 2 to 5 cm 3.8 times the size of the tumor patients who have less than 2 cm(P=0.009). Patients who had 3 to 6 lymph nodes involved  were 3.1 times  those less than 2 lymph nodes have metastasis(P=0.003).Also grade of tumor and HER-2 were significant to metastasis(P=0.04).
Conclusion: Using illness-death model that is suitable for the analysis of such data, It was found that lymph nodes involved and size of tumor metastasis were significant. So it need to early detection to prevent metastasis and death is felt.
